Position Summary
We are seeking a compassionate, detail-oriented, and independent Registered Nurse (RN) to join our home care agency on a per-case basis as a Clinical Supervisor. This flexible, field-based position offers the opportunity to play a foundational role in shaping our agency’s quality of care and operational success.
The position has the potential to transition into a full-time clinical leadership role as our client base grows.
The RN will conduct in-home client evaluations, develop individualized service plans, train and assess caregivers, and perform monthly follow-up evaluations for active clients. Ideal candidates are experienced home care nurses who value clinical autonomy, flexible scheduling, and meaningful patient impact.
Compensation

Initial In-Home Client Evaluation: $120 per completed visit

Monthly Re-Evaluation Visit: $70 per visit

Caregiver Training & Skills Evaluation: $45 per hour
- Compensation is per completed service; no guaranteed minimum hours
Key Responsibilities
- Conduct comprehensive in-home assessments for prospective clients
- Develop and maintain individualized service and care plans
- Perform monthly supervisory visits to review care, monitor health outcomes, and ensure care plan adherence
- Provide initial and ongoing training for Home Health Aides (HHAs), including condition-specific competencies
- Document all client visits, trainings, and evaluations in accordance with agency and regulatory requirements
- Serve as a clinical liaison to caregivers and families, addressing questions or changes in condition
- Ensure all services comply with New Jersey state home care regulations
- Contribute to agency development, operational processes, and community outreach during the launch phase
Qualifications
- Active Registered Nurse (RN) license in the State of New Jersey (Required)

Bachelor of Science in Nursing (BSN) preferred
- Minimum 3 years of experience in home care or community-based nursing (Required)
- Knowledge of New Jersey home care standards and documentation protocols
- At least 21 years of age
Skills & Competencies
- Strong clinical assessment and judgment
- Clear verbal and written communication skills
- Comfortable providing caregiver education and clinical feedback
- Organized, detail-oriented, and able to manage multiple cases
- Proficient in Microsoft Office and digital charting/documentation systems
- Self-motivated with the ability to work independently
Physical Requirements
- Able to travel to client homes and agency office as needed
- Able to sit, stand, and use a computer for extended periods
- Able to lift up to 15 pounds occasionally
